Documents Required for Document Verification (DV)

Candidates must bring all original documents along with a complete set of self-attested/attested photocopies on the day of the interview. Ensure all certificates are in the ‘prescribed proforma’ where specified.

  • Physically Handicapped (PH) Certificate: In the “prescribed proforma” issued by the competent authority, if applicable.
  • Documents for Change of Name: (if applicable) For women claiming change of name due to marriage/remarriage/divorce, or for any other circumstances for male and female, as detailed in Annexure-I of the notification. This includes divorce deeds, marriage certificates, affidavits, newspaper cuttings, or Gazette Notification.
  • Age Relaxation Certificates: In “prescribed proforma” for Ex-Servicemen, Central/UT Government Employees, Meritorious Sports persons, Widows/Divorced Women, or persons domiciled in J&K during specified periods.
